- Przedmiotem zamówienia jest dostawa, instalacja i uruchomienie trombektomu mechanicznego, zwanego dalej „sprzętem” oraz dostawa cewników, zwanych dalej „wyrobami” zamawianych partami według aktualnych potrzeb Zamawiającego. Ilości wyrobów podane w formularzu cen jednostkowych mają charakter orientacyjny i Zamawiający ma prawo zamówić ich mniejsze ilości, z tym, że nie mniej niż 80% wartości umowy na wyroby. 2. Opis przedmiotu zamówienia został zawarty w formularzu „Parametry wymagane” (zał. 2 do SWZ, zwany dalej również jako „FPW”) oraz w formularzu cen jednostkowych (zał. 2.1. do SWZ, zwany dalej również jako „FCJ”), formularze te winny bezwzględnie być złożone wraz z formularzem oferty (zał. 1 do SWZ, zwany dalej również jako „FO”).
