Scope overview
District is requesting Internet service (IA and transport) for students and staff located at 1001 Touhy Avenue, Des Plaines, IL 60018. School also willing to consider fiber option as noted in service requests. Multi-year contracts should have option for increased bandwidth at commensurate price increase. Please give pricing in increments of 100 Mbps.
All proposals and questions must be submitted via email only to the main contact and the technical contact listed on this form 470. Vendors must have a valid E-Rate SPIN and current SPAC to be considered. All responding vendors must meet certification requirements prohibiting the inclusion of any products or components from Huawei, ZTE, or any other covered company designated by the FCC.
We reserve the right to reject proposals from vendors who do not meet these criteria. District also reserves the right to disqualify incomplete vendor proposals. Please review your quote prior to submittal to ensure that you have complied with the form 470 requests. Failure to comply with these instructions may result in disqualification of vendor proposals.
