School's current IA service(s) include: 1 line @ 1GB DIA Fiber (EXPIRING agreement) at associated BEN address: School has an expiring agreement for listed Fiber Internet service.

Bidding for Fiber Starting at the current 1Gb Speed, 2Gb, and tiers to 5Gb is requested. Note: form 470 line item may list a higher maximum speed -in the event modifications are needed during a Multi-Year-Agreement. Note: Any fiber proposal must include a service delivery viability confirmation prior to submission. The School/District may prioritize a relationship with an internet provider to a reseller.

SPI discounting method will be designated upon any agreement unless specifically requested during the funding year by the school. SPI submission by the provider will be implied upon submission of a bid. Listed number of lines under Service Requests: Category One is current assessment and should be considered the minimum required.

The Minimum/Maximum speeds requested should be presented with available options- from lowest to highest speeds. Final contract should include option for coterminous upgrade tiers listed in this 470. Please include per dedicated IP block charge if not included in the internet bundle. An Additional DTS line quantity may be utilized and considered part of this request.

Entity reserves the right to choose to purchase ANY listed 2026 form 470 ISP, Transport, and Internet services bundled OR unbundled, in addition to initial configuration and support (including bundled managed router/firewall, IP addresses and related misc. eligible C1 services).

Per USAC sunset guidelines on eligible services- service providers may wish to include an additional per line voice cost in your proposal, as questions about the number of voice lines will not be considered part of this request and may receive no additional response. A lower download speed than current IA service may be determined as superior over current connection method [ex: Dedicated Fiber over Cable Modem].

The following items are required to be considered a valid bid: (1) Initial contact MUST be made VIA EMAIL ONLY, to the Email address listed in this form 470 form 470@ERATESUPPORT.COM. (2) The email subject line must reference the SCHOOL NAME and 470 NUMBER. (3) All responses MUST reference the services that you are bidding on. (4) All bids must contain your SPIN, Company Name, and Contact Information.

(5) provider questions must be for items not specified in this narrative; redundant requests for information and sales emails for other items will be ignored. (6) bids must comply with all applicable state/ local procurement laws, rules, and contract regulations. accepted bids will be subject to E Rate funding and Board approval. School will consider multi-year contracts or contracts with a voluntary extension.

Proposals may be presented with a single-year option up to a 5-year/multi-year option. Contracts are contingent upon eRate approval and support unless otherwise noted and may be cancelled at any time.

Unless otherwise noted, the initial proposal submission deadline is considered PRIOR to the Allowable Contract Date noted in this Form 470- Initial bidder contact ON or AFTER the 28-day window for Allowable Contract Date may not be accepted. NO LATE BIDS will be considered unless modifications to the ACD deadline are subsequently executed.

For security purposes, emailed links to an external site outside of the contents of an email may not be considered a valid bid, unless otherwise noted. Any bid that fails to conform to the essential requirements of the invitation for bids, or contains basic and/or irrelevant answers, may be rejected.

Likewise, multiple redundant auto-generated or blanket bids from a single vendor may be disqualified, and/or only the final submitted bid be considered. Revised bids to submissions received during the initial window may be submitted during the 28-day bid window and subsequent vetting period where necessary, the last version will be considered the final document.
