Etap II: Zakup sprzętu ICT. Etap III: Digitalizacja dokumentacji medycznej. Szczegółowy opis przedmiotu zamówienia, został określony w Załączniku nr 2, na który składają się następujące dokumenty: Sposób realizacji i terminy płatności zostały określone w projekcie umowy, który stanowi Załącznik nr 3 do SWZ.
