Rock Falls SD 13 is seeking proposals for a Multi-Point to Point 10-Gbps WAN to connect its Headend Dillon Elementary School: 815-625-3356, 1901 Eighth Ave., Rock Falls, IL 61071 in the MDF server room to Rock Falls Middle Schools' MDF Room at 1701 Twelfth Ave., Rock Falls, IL 61071 and to connect to the Merril Elementary School 815-625-4634, 600 Fourth Ave., Rock Falls, IL 61071 in its MDF Room.
Please provide pricing for 1-year managed lease terms. Proposals are due by Tuesday, March 31, 2026 by 12:00 noon CST. DISQUALIFICATION FACTOR: Responses not received by Due Date cited in the Form 470 narrative may be disqualified. DISQUALIFICATION FACTOR: Responses that do not address the requirements in the Form 470 will be disqualified. DISQUALIFICATION FACTOR: Responses that do not provide the Lowest Corresponding Price will be disqualified.
Service providers submitting proposals in response to this FCC Form 470 must be in compliance with rules and orders governed by the Federal Communications Commission. The vendor must agree to comply with rules and regulations of USAC. Vendors must be familiar with billing methods and timelines associated with USAC. Vendor must be in good standing with the USAC and not red-lighted.
The vendor must include their SPIN number on the bid/proposal. Service providers responding to this Form 470 should clearly identify E-rate Eligible and Ineligible equipment and pricing. Any ineligible products, services or components bundled with eligible products or services must be listed separately from the eligible products or services.
Prices for these ineligible products or services must be allocated out of the price for the eligible services; failure to do so can lead to rejection of the bid. Allocations must comply with USAC rules which specify that the allocation methodology must be based on tangible criteria and reach a realistic result.
It is the Service Providers responsibility to access the Portal (EPC) to determine if there are any addenda or changes for this request. There are no Services Requested For Category 2. Agency: Rock Falls Elem School Dist 13 Status: Certified
