(ITP), 2740 PINE GROVE AVE. PORT HURON, MI 48060, PLANS AND MANAGES A TELEPHONE CAMPAIGN FOR PUBLIC SUPPORT AND AWARENESS. UNITED CANCER SUPPORT FOUNDATION (UCSF) IS TO RETAIN 10% OF THE GROSS AMOUNT OF CONTRIBUTIONS. AS PAYMENT FOR ITS SERVICES, ITP SHALL RECEIVE THE REMAINDER 90%. ITP DOES NOT HAVE CUSTODY OF FUNDS; ALL CONTRIBUTIONS ARE SENT TO UCSF.
