Scope overview
Dit is een vooraankondiging. Dienstverlening WMO accent is een aanvulling op de op de reguliere toegangs- en dienstverleningsketen in de Wet maatschappelijke ondersteuning (Wmo).
De dienst bestaat uit multidisciplinaire begeleiding aan de gebruiker bij de aanvraag van verschillende benodigde vormen van voorzieningen enerzijds en het organiseren van een coherente samenhang tussen die verschillende types dienstverlening anderzijds. Ook kan het uitbrengen van indicatie advies tot de dienstverlening behoren.
Dienstverlening Wmo Accent is in het leven geroepen voor een groep Amsterdammers, die beperkt door GGZ-problematiek en/of complexe meervoudige fysieke kwesties, niet in staat is om zelfstandig de benodigde aanvraagtrajecten te doorlopen.
Ook kan het Amsterdammers betreffen waarbij uitgebreider onderzoek wordt vereist dan in de reguliere procedure wordt geboden om een goed beeld te verkrijgen van de medische situatie en de daaruit voortvloeiende aanvraag.
