Przedmiotem zamówienia jest wsparcie digitalizacji dokumentacji medycznej, tj. dostawa wraz z wdrożeniem Oprogramowania Aplikacyjnego obejmująca: A) Wykonanie Wdrożenia Oprogramowania Aplikacyjnego, B) Udzielenie licencji na Oprogramowanie Aplikacyjne. Szczegółowy opis przedmiotu zamówienia stanowi Załącznik nr 1 wraz z Załącznikiem od A do C do projektowanych postanowień Umowy.
Wymagania dotyczące realizacji Umowy zostały opisane w Załączniku od nr 1 do 6 do Umowy, które stanowią integralną część Umowy. Wykonawca zobowiązany jest zrealizować zamówienie na zasadach i warunkach zawartych w projektowanych postanowienia umowy w sprawie zamówienia publicznego, które zostaną wprowadzone do treści tej umowy i określone zostały w załączniku nr 2 do SWZ.
