REPLACE iDXA C211 FOR THE VETERANS AFFAIRS MEDICAL CENTER (VAMC) CINCINNATI, OH PROJECT NUMBER: 539-26-505 QUESTIONS AND ANSWERS (RFIs) TO REQUEST FOR QUOTE (RFQ) NUMBER 36C25026Q0375 QUESTIONS AND ANSWERS RFI #1 Question: Given the size of the project, is a dedicated SSHO and the CQ education requirements required?

Typically, the Superintendent can be the SSHO and CQC on this size of project, and a superintendent is not going to be a graduate engineer. 1.9 A - 01 35 The Prime Contractor shall provide a minimum of one Full-Time SSHO at each project site, for each shift (with no other duties) that holds as current, a professional safety certification with at least 3 years of dedicated construction safety related experience.

3.4 B 01 45 The CQC system Manager is required to be a graduate engineer, graduate architect, or a graduate of construction management, with a minimum of PM or SRE to determine qualifications based on project complexity at construction review 5 years construction. Answer: The superintendent may serve as the SSHO and CQC System Manager. A graduate degree is not required.

SSHO must be 30 hours of OSHA training (see spec 01 35 26 parts 1.5F). RFI #2 Question: The solicitation and drawings reference Room C211, while portions of the specifications reference C214. Could you please confirm the correct room designation and whether all documents apply to C211?

Answer: The majority of contract documents, including the Statement of Work and full drawing set, identify Room C211 as the correct project location. The specifications, however, contain an incorrect header reference to C214. This appears to be an error in the header only; the specifications do not assign any scope to C214.

RFI #3 Question: The SOW indicates that hazardous material testing and abatement will be performed if required. Has a hazardous materials survey already been completed? Should bidders include abatement costs in the base bid, or will this be handled separately by the owner? Answer: The Statement of Work requires the contractor to perform hazardous materials testing and abatement if required.

No hazardous materials survey is included in the solicitation, and bidders are not directed to include abatement in the base bid. Contractors should therefore include testing only in their proposals. If hazardous materials are identified during construction, the VA will address any required abatement separately at that time. RFI #4 Question: The drawings don t describe anything about the fume hood for disconnecting.

There is power and some other type of gas or air ports and controls on the hood. We need to know how the hood is connected and what is connected to it. Is there piping or conduit behind the hood to be capped and or removed. Is there a drawing of the hood to reference connections?

Answer: There are no existing as-built or construction drawings for this fume hood, and its installation date and project of origin are unknown. The fume hood has electrical power and medical gas connections. These utilities must be disconnected and capped. RFI #5 Question: Is there a fan on the roof? If so, does that need to be removed along with any duct in the walls?

Answer: There is no roof-mounted fan associated with this fume hood. The hood is not currently connected to any exhaust fan and does not have ductwork running to the roof or through the walls that would require removal therefore no fan removal is required. See picture below of disconnected unit.
